The Municipality of Alilem is one of the fourteen interior towns situated second at the southernmost part of Ilocos Sur. It lies between 16˚52’ north latitude and 120°40’ east longitude. It is bounded on the north by the Municipality of Suyo, Ilocos Sur on the south by the Municipalities of Sugpon, Ilocos Sur and Bakun, Benguet, on the west by the Municipalities of Tagudin, Ilocos Sur and Sudipen, La Union and on the east by the Municipalities of Cervantes, Ilocos Sur and Bakun, Benguet.

The town is eleven (11) kilometers from the national highway via Sudipen, La Union; 12.6 kilometers from Tagudin, Ilocos Sur; 46.0 kilometers away from Candon, Ilocos Sur; 107.0 kilometers from Vigan, Ilocos Sur the provincial capital of Ilocos Sur; 58.0 kilometers from San Fernando, La Union; and 316.0 kilometers away from Manila.

The municipality particularly barangays Anaao, Poblacion, Dalawa and Amilongan can be reached in and out by motor vehicles year-round however, the remaining barangays can only be reached thru motor vehicles during the dry season passing through Sudipen, La Union and Suyo, Ilocos Sur with an estimated distance range of 11.0 – 15.0 kilometers.
