  • Kindergarten (Age 5)
  • Elementary (Age 6-11 years old)

School Availability of the Service

  • Kindergarten and Elementary: Two (2) weeks before the start of the academic school year.

Service Duration

  • Half-Day

Requirements for Enrollment: For Kindergarten and Elementary:

  • Report Card for Grade II to VI
  • Birth Certificate for Kindergarten and Grade I Pupils For Transferees:
  • Certificate of Good Moral Character

ISSUANCE OF FORM 137/N.A.T RESULT/GOOD MORAL CHARACTER

Schedule of Availability of the Service:

  • Monday and Friday from 8:00 AM to 5:00 PM (No noon break)

Who may avail of the service?

  • Grade I to VI pupils/students/transferees

Requirement to avail the service:

  • Accomplished request slip

FILLING OF COMPLAINTS

Requirements:

  • Full name and address of the complainant
  • Full name and address of the person complained of, along with their position in the school
  • A narration of relevant and material facts showing the acts or omissions allegedly committed by the person
  • Certified true copies of documentary evidence and affidavits of witnesses, if any
  • Complaint shall be under oath

How to avail the service?

  • File the complaint along with the requirements at the office of the Principal
  • Service Provider receives the complaint and other documents
