• Photocopy of PSA Birth Certificate
• Registration Fee of P 500

Requirements for Transferees:

• Form 138 (Report Card)
• 2 pieces of 2×2 ID pictures with a white background
• 1 photocopy of PSA Birth Certificate
• Registration Fee of P 500

Junior High School

• Form 138: If not available, a Certificate of Grade 6 Enrollment can be provided.
• 2 pieces of 2×2 ID pictures with a white background
• 1 photocopy of PSA Birth Certificate
• Good Moral Certificate
• Registration Fee of P 500

Senior High School

• Form 138: If not available, a Certificate of Grade 10 Enrollment can be provided.
• 2 pieces of 2×2 ID pictures with a white background
• 1 photocopy of PSA Birth Certificate
• Good Moral Certificate
• Registration Fee of P 500

College Department

• Form 137 or 138: If not available then Cert of Grade 12 Enrollment
• 2pcs 2×2 ID picture with white background
• Original copies of Certificate of Residency
• 1 photocopy of PSA Birth Certificate for single or Marriage Certificate for Married
• Good Moral
• Registration Fee of 1,000 pesos.
• Also provide gmail account and mother’s maiden name upon enrollment.
